Case transfer between specialists: How do you test continuity of follow-up?

How do you test case transfer between specialists in the consulting system? Plan delivery, tasks, appointments, receipt acceptance, and access limits after conversion.

What this guide covers
  1. Why is this requirement included in the purchasing decision?
  2. What should the scope cover?

  5. The next step is to document the requirements

Transfer is a moment when situations usually lose their context: the file is transferred, but the knowledge is not transferred. The new specialist starts from scratch and repeats what happened, and the client feels that he is repeating his story from the beginning – and this alone is a common cause of interruption.

Why is this requirement included in the purchasing decision?

Because a transfer inevitably occurs: a specialist leaves, the case requires a different specialty, or the client requests a change. The way the system deals with it determines whether the follow-up continues or starts again.

It also has a privacy aspect: after the conversion, what is the former specialist still able to see?

What should the scope cover?

  • Documented referral with a reason: Is created as a due request, and is approved according to your policy — usually by your administrator.
  • Full context transition: The episode with its history, evaluations, plan, tasks, and upcoming dates.
  • Delivery summary: A field in which the previous specialist writes what the new specialist needs to know — which conveys knowledge, not just data.
  • Accept receipt: The new specialist confirms that he has received the case, so it does not remain suspended between the two.
  • Transfer validity: The previous loses knowledge of developments, and the new gains.
  • History remains in his name: What was documented by the previous one remains attributed to him and is not attributed to the new one.
  • Client Notice: In the format approved by your facility.

Checklist in demo

  • Create a transfer: Was a reason requested? Do I need accreditation?
  • Open the case with the new specialist’s account: Did the plan, tasks and appointments appear complete?
  • Read delivery summary: Is there a custom field for it?
  • Log in to the previous specialist’s account: Does he still see updates? Expected: No.
  • Open an old note: Is it still in its original author’s name?
  • Request a report on transfers and their reasons for a period.

The most important element of the whole process is the delivery summary: a few lines written by the previous specialist about what was tried, what worked and what should be avoided. Data alone does not convey this knowledge, and reading ten previous notes does not compensate for it. Require this field and make it mandatory for conversion.

Limits that need to be adopted

  • Transfer is a professional decision: Is not executed automatically for administrative reasons such as load balancing. The therapeutic relationship is part of the service.
  • Limited information after conversion: The previous may need to be viewed for supervisory or inquiry purposes — temporarily granted with registered authority that does not remain open.
  • Client consent: His notice and reason for the transfer — and in some cases his approval — is part of your policy, not a technical procedure.
  • Diagnostic indicator: The large number of transfers in a particular service indicates a defect in the sorting that deserves an administrative review.

The next step is to document the requirements

Write your conversion path: who requests, who approves, and what is written in the delivery. See Counseling centers page.
