Session Management & Consultation Documentation

Document sessions and consultations with flexible templates, and manage review, signing, tasks and follow-up. Documentation is not a formality: the specialist’s time is the most valuable resource in your center, and the quality of what is written after the session is what your facility will rely on in any professional review, complaint or audit.

Preparing for the session from a clear context

Before entering the session, the specialist needs two minutes, not a quarter of an hour, to search various files. The system displays the case summary, latest sessions, assessments, goals, and tasks on one screen linked to the appointment itself. This has an operational impact: sessions start on time, and a specialist is not exhausted in research before each meeting.

Case Summary Referral reason and date of the current episode and previous archived episodes.
Latest sessions Notes of the previous two sessions and the interventions decided in them.
Evaluation results Another Periodic evaluation And its path compared to the previous one.
Active targets Objectives Plan And the status and stage of each target.
Tasks between sessions What was previously assigned to the client and what he actually accomplished before this meeting.
Patron Updates What he has recorded in the diary or sent through the portal since the last session.
Valid approvals Service approvals, reporting sharing and approved disclosure limits.
Financial situation The remaining sessions of the package or the insurance entitlement of the case.

Documentation that matches the entity’s procedures

Each center has its own style of documentation, and each session type has its own requirements. The system provides general and specialized templates and custom fields for notes, interventions, assignments, and next session plan. What is required is for the system to adapt to your team’s methodology, not for your team to reshape its work to fit an existing model.

Templates by session type

First consultation, individual follow-up, family session, children’s session, remote consultation — each type has its own template and fields.

Custom fields

Add the structured, reportable fields your facility needs, instead of cramming everything into a free text box.

Interventions and duties

Recording what was implemented in the session and what was assigned to the client afterward, to be transformed into follow-up tasks related to the plan.

Next session plan

He writes down what the next meeting will be based on, and the specialist finds it ready in his preparation without relying on memory.

Attachments

Reports, referrals, and documents are uploaded within the session and remain linked to it and the client’s file.

Multilateral sessions

In a family or marital session, what belongs to the group is separated from what belongs to each individual, in a way that protects the privacy of the parties.

From draft to approval

The observation goes through a clear path: a draft is saved, sent for review if required by center policy, and approved with the specialist’s signature or a joint signature with the supervisor. After adoption, nothing is erased — added. This is exactly what makes documentation valid to be invoked.

1

Draft

The specialist writes during or immediately after the session, and saves it without obligation. The draft is visible to him alone.

2

Supervisor review

Sent for review when required by center policy — for example, for new specialists or for specific session types.

3

Accreditation and signature

The specialist approves it with his signature, or with a joint signature with the supervisor, and the date of documentation and the identity of the signatory are preserved.

4

Later addition

Any correction or completion is recorded as an addition documented by its date and author, and the original text remains as it is.

Authentication timeout Operational setting No Just a reminder. Your facility determines the period allowed to approve the note after the session, and the system displays the undocumented sessions to the supervisor so that they do not accumulate. Documentation weeks late does not help the case and does not protect the facility.

What do you test before you buy?

In the demo, order one continuous journey: start a session from an appointment, document it, approve it, assign a task to the client, then book the follow-up appointment — without entering duplicate data and without exiting to another screen. This journey reveals in minutes what a ten-page list of advantages does not reveal.

Marks a system worth buying

What you should see as you watch the journey.

  • The session starts from the date itself, not by manually creating a new record.
  • The client’s name, status, and goals are preloaded in the note.
  • An assignment recorded in the session becomes an assignment in the plan without rewriting.
  • The follow-up booking from within the session appears immediately in the calendar.
  • Documentation is completed in a few minutes after the meeting ends.

Signs to stop

What later costs you your team’s time every day.

  • Re-enter the client name or service type in each screen.
  • Documentation is in a single free text box with no structured fields.
  • Possibility to modify an approved note without trace in the record.
  • There is no audit trail or shared supervisor signature.
  • Tasks and appointments are disconnected from the session screen.

Calculate it with your team’s time

This difference translates into a number that you can estimate yourself before hiring: multiply the number of your monthly center sessions by the minutes the specialist spends today on research, documentation and re-entry. Then ask in the presentation:

  • How many minutes does it take to observe a complete follow-up session from opening to approval?
  • How many clicks does it take to go from session to follow-up booking?
  • How does the administrator know the number of unauthenticated sessions today?
  • What happens if the connection is lost while writing a note?

Review your team’s documentation requirements

Project scope cannot be accurately estimated without knowing how your team documents today. Define these elements for us before the presentation, and we prepare the system with your templates – so you see your team working on their actual screens, not on a demo model.

What affects the scope of implementation?

The number of session types and templates, the depth of custom fields required, the multiple approval paths between teams or branches, and whether you have a written documentation guide must be reflected in the system verbatim. All items are clear View price No open estimates.

Have these items ready before requesting the offer

Send it with you to the meeting, or attach it to your demo request form:

  • The types of sessions your center offers and the duration of each type.
  • One or two forms of documentation notes currently in use.
  • Approval path: who reviews, and when a co-signer is required.
  • The time limit you use to document the session after it ends.
  • Any requirement of a licensor or contracting authority regarding the form of documentation.

Questions about documentation and review

Three questions that specialists and supervisors ask before adopting the system.

Can the documentation template be customized?
Yes, and each session type has a separate template. Your facility determines which template sections and fields are mandatory, and adds structured custom fields that later appear in reports. Templates are built within the implementation setup based on your existing templates, and can be modified after launch. Modifying a template does not change the notes written in the previous version, so the old records remain as they were documented.
Can a supervisor review notes?
Yes, within his authority over his team’s cases. Your facility determines when review is mandatory: for all sessions, for specific types, or for supervised professionals during a specific period. The supervisor can return the observation to the specialist with his notes, or approve it with a joint signature. Every information or action is recorded in Audit log, the professional review remains documented, not verbal.
How do you correct a note after it has been approved?
With a documented addition, not a silent modification. The approved text remains as it is, and the correction or completion is recorded as a later addition with its date, the identity of its author, and the reason for it, so the record is read in chronological order. This principle is intentional: a system that allows an approved note to be changed without trace does not serve as evidence in any review or dispute — which is the first thing you should test in the demo.

Watch your team document a real session

Submit one documentation template that you use, and we prepare it within the system before display. You’ll see the complete journey with your templates, session types, and certification path — and measure for yourself the time you save on each specialist per day.

  • Your Templates You are not a public demo template.
  • Audit and signature path as approved by your facility.
  • Patch testing after certification is right in front of you.
